Dear Members,

We are issuing KAVACH cards to all registered members, in order to issue the same, please fill in all the details as required in attached form mandatorily. Please note each member has to fill individually i.e. 
Husband, Wife and Kids  and parents if all members,  must fill it separately. So each person one form 

These details are mandatory to generate the KAVACH card. Failure to provide the info will delay your card delivery to you. 

Please note, insurance for those who have requested shall be added later, once we get the details to share to those who want to enroll under group insurance. For this card shall be given to all registered members to avail the services and those who opt insurance with it shall be linked with it. *PLEASE FILL
